trail.
Termination of Stay:
This form needs to be filled out if you are leaving UCO/Lick for
greener (or otherwise) pastures. If you do not fill out and return
this form to the Business Office (LOBO), we won't know that you're
leaving. If we somehow know that you are leaving, we'll still
ask you to fill out this form.
UCO/Lick Computer Account Application: We hope to have this
form online soon. You need to have this form filled out and signe
by your PI (the one with the signature authority for the money) and
retuned to LOBO in order for NICS to create your computer account.
You can pick up a copy from the NICS offices in the first floor of
Nat Sci II, or over at LOBO in the ISB.
Order memo: This form is a multi-sheet carbon copy
form. Please pick one up from either the NICS offices in Nat Sci II,
or at LOBO.
